Explanation of Benefits

BAC’s Explanation of Benefits (EOB) is now a monthly statement.

The EOB explains how medical claims were processed during the previous month. It shows what the provider charged, any discounts applied, what the plan paid, and what you may owe the provider. An EOB is not a bill. It is for your information and records.

Participant Information:

Group Name – Employer Health Plan

Group Number – Plan ID shown on your BAC ID Card.

Member Name – Employee Name unless dependent is over 18 then they receive their own statement.

Subscriber ID – ID Number from your BAC ID Card

Date – Claims shown were processed during the previous month. Services may have occurred earlier.

Claim Number

Your unique claim reference number. Please have this available if you call BAC with questions.

Provider/Patient No.

The doctor or facility that submitted the claim and the number assigned to your record from that provider or facility.

Claim Info:

Dates of Service – When the doctor visit occurred.

Service Code – Standard billing codes that describe the medical services you received. Code descriptions are listed at the end of the EOB.

Amount Billed – What the provider originally charged for this service.

PPO Discount – A network discount applied by your health plan. You do not owe this amount.

Deductible / Copay / Coinsurance – Your share of the cost, if applicable, under your plan.

Plan Payment – The amount your health plan (employer) paid for this claim.

Patient Responsibility – The amount you may owe your provider. A bill received from your provider should match this amount.

Comments / Reason Codes

Explains discounts or adjustments such as PPO savings or duplicate charges.

Monthly Statement Summary

A summary of all claims processed during the previous month summarized by provider.

When viewing claims in the member portal, a Temporary EOB with basic claim information will appear until the monthly statement is generated.

Enroll in paperless EOBs today!

Once logged into the BAC Member’s Area, select ‘My Account’ in the top right corner and then select ‘Manage Account’ and change your EOB Delivery Settings to Go Paperless.
